Description:
The Senior Outreach and Care Management Program is committed to helping seniors maintain their health and live independently in their own homes for as long as safely possible. Eligibility: Services are provided for those residing in Passaic County whose 60 and over. In home interviews are conducted and seniors are assessed for needs than linked to Community Services, care management provided when needed.
